City Office of Chief Medical Examiner (OCME) was reviewing more than 800 rape cases in which a technician, now known to be Serrita Mitchell, may have mishandled evidence, which affected criminal investigations. The N.Y. State Inspector Gen. (IG) opened an investigation into the allegation. On April 19, 2013, Theresa Caragine, Deputy Dir. of the Dept. of Forensic Biology within the OCME, resigned her position for allegedly failing to follow lab protocol. The IG expanded the investigation to include this alleged failure. This report of the IG investigation determined that the OCME employed Serrita Mitchell for 10 years despite consistent poor reviews and underperformance. It also confirms that OCME's Deputy Dir. failed to follow protocol for resolving disputes over scientific findings. More significantly, errors by the analyst and the misplacement of items in her cases went undetected for the same period. On Jan. 20, 2011, the analyst was permanently suspended from casework.
