saltzer lab hours nampa: Big Med David Dranove, Lawton Robert Burns, 2022-11-18 When it comes to healthcare, bigger isn't always better. The early-1990s rise of megaproviders--large, hospital-based healthcare systems that have become the norm in American medicine--brought promises of accessibility, cost savings, and excellence to the American healthcare experience. Today's megaproviders, following three decades of growth and consolidation, receive as much as two-thirds of healthcare spending in the United States. Big Med examines the rise of these megaproviders and their formative role in reducing American healthcare to its current shambles. As healthcare organizations have consolidated, they've increased their market power, and in doing so created a system in which the network sets the prices, insurance and pharmaceutical companies take the blame, and Americans suffer the costs. Drawing on seven decades of combined research in economics and sociology, Dranove and Burns consider the effects of this noncompetitive system on patients, doctors, and society more broadly. Physicians are forced to provide less attentive care to a larger number of patients; patients in turn pay more for lesser care. This leaves both parties alienated and disenchanted, and any motivations to improve the flawed system are stalled. Amid screeching public debate around the prospects and perils of Medicare-for-all, Big Med is a provocative, narrative-shifting account of who's really calling the shots--and causing harm--in American healthcare today-- |

saltzer lab hours nampa: If I Betray These Words Wendy Dean, Simon Talbot, 2023-04-04 An incredibly important and captivating book for patients, families, and clinicians detailing how we’re all hurt by corporate medicine “Wendy Dean diagnoses the dangerous state of our healthcare system, illustrating the thumbscrews applied to medical professionals by their corporate overlords… Required reading for all stakeholders in healthcare.” — Danielle Ofri, MD, PhD, author of When We Do Harm: A Doctor Confronts Medical Error Offering examples of how to make medicine better for the healers and those they serve, If I Betray These Words profiles clinicians across the country who are tough, resourceful, and resilient, but feel trapped between the patient-first values of their Hippocratic oath and the business imperatives of a broken healthcare system. Doctors face real risks when they stand up for their patients and their oath; they may lose their license, their livelihood, and for some, even their lives. There’s a growing sense, referred to as moral injury, that doctors have their hands tied – they know what patients need but can’t get it for them because of constraints imposed by healthcare systems run like big businesses. Workforce distress in healthcare—moral injury—was a crisis long before the COVID-19 pandemic, but COVID highlighted the vulnerabilities in our healthcare systems and made it impossible to ignore the distress, with 1 in 5 American healthcare workers leaving the profession since 2020, and up to 47% of U.S. healthcare workers now planning to leave their positions by 2025. If I Betray These Words confronts the threat and broken promises of moral injury – what it is; where it comes from; how it manifests; and who’s fighting back against it. We need better healthcare—for patients and for the workforce. It’s time to act. |

saltzer lab hours nampa: Care Without Coverage Institute of Medicine, Board on Health Care Services, Committee on the Consequences of Uninsurance, 2002-06-20 Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash. |

saltzer lab hours nampa: The American Health Care Paradox Elizabeth H. Bradley, Lauren A Taylor, 2013-11-05 Foreword by Harvey V. Fineberg, President of the Institute of Medicine For decades, experts have puzzled over why the US spends more on health care but suffers poorer outcomes than other industrialized nations. Now Elizabeth H. Bradley and Lauren A. Taylor marshal extensive research, including a comparative study of health care data from thirty countries, and get to the root of this paradox: We've left out of our tally the most impactful expenditures countries make to improve the health of their populations-investments in social services. In The American Health Care Paradox, Bradley and Taylor illuminate how narrow definitions of health care, archaic divisions in the distribution of health and social services, and our allergy to government programs combine to create needless suffering in individual lives, even as health care spending continues to soar. They show us how and why the US health care system developed as it did; examine the constraints on, and possibilities for, reform; and profile inspiring new initiatives from around the world. Offering a unique and clarifying perspective on the problems the Affordable Care Act won't solve, this book also points a new way forward. |

saltzer lab hours nampa: The Role of Telehealth in an Evolving Health Care Environment Institute of Medicine, Board on Health Care Services, 2012-12-20 In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment. |
saltzer lab hours nampa: Big Data, Health Law, and Bioethics I. Glenn Cohen, Holly Fernandez Lynch, Effy Vayena, Urs Gasser, 2018-03-08 When data from all aspects of our lives can be relevant to our health - from our habits at the grocery store and our Google searches to our FitBit data and our medical records - can we really differentiate between big data and health big data? Will health big data be used for good, such as to improve drug safety, or ill, as in insurance discrimination? Will it disrupt health care (and the health care system) as we know it? Will it be possible to protect our health privacy? What barriers will there be to collecting and utilizing health big data? What role should law play, and what ethical concerns may arise? This timely, groundbreaking volume explores these questions and more from a variety of perspectives, examining how law promotes or discourages the use of big data in the health care sphere, and also what we can learn from other sectors. |

saltzer lab hours nampa: No Accident Neil Arason, 2014-04-29 It is possible to eliminate death and serious injury from Canada’s roads. In other jurisdictions, the European Union, centres in the United States, and at least one automotive company aim to achieve comparable results as early as 2020. In Canada, though, citizens must turn their thinking on its head and make road safety a national priority. Since the motor vehicle first went into mass production, the driver has taken most of the blame for its failures. In a world where each person’s safety is dependent on a system in which millions of drivers must drive perfectly over billions of hours behind the wheel, failure on a massive scale has been the result. When we neglect the central role of the motor vehicle as a dangerous consumer product, the result is one of the largest human-made means for physically assaulting human beings. It is time for Canadians to embrace internationally recognized ways of thinking and enter an era in which the motor vehicle by-product of human carnage is relegated to history. No Accident examines problems related to road safety and makes recommendations for the way forward. Topics include types of drivers; human-related driving errors related to fatigue, speed, alcohol, and distraction and roads; pedestrians, cyclists, and public transit; road engineering; motor vehicle regulation; auto safety design; and collision-avoidance technologies such as radar and camera-based sensors on vehicles that prevent crashes. This multi-disciplinary study demystifies the world of road safety and provides a road map for the next twenty years. |

saltzer lab hours nampa: Clay Water Brick Jessica Jackley, 2015-06-23 In the tradition of Kabul Beauty School and Start Something That Matters comes an inspiring story of social entrepreneurship from the co-founder of Kiva, the first online microlending platform for the working poor. Featuring lessons learned from successful businesses in the world’s poorest countries, Jessica Jackley’s Clay Water Brick will motivate readers to more deeply appreciate the incredible entrepreneurial potential that exists in every human being on this planet—especially themselves. “The heart of entrepreneurship is never about what we have. It’s about what we do.” Meet Patrick, who had next to nothing and started a thriving business using just the ground beneath his feet . . . Blessing, who built her shop right in the middle of the road, refusing to take the chance that her customers might pass her by . . . Constance, who cornered the banana market in her African village with her big personality and sense of mission. Patrick, Blessing, Constance, and many others are among the poorest of the world’s poor. And yet they each had crucial lessons to teach Jessica Jackley—lessons about resilience, creativity, perseverance, and, above all, entrepreneurship. For as long as she could remember, Jackley, the co-founder of the revolutionary microlending site Kiva, had a singular and urgent ambition: to help alleviate global poverty. While in her twenties, she set off for Africa to finally meet the people she had long dreamed of helping. The insights of those she met changed her understanding. Today she believes that many of the most inspiring entrepreneurs in the world are not focused on high-tech ventures or making a lot of money; instead, they wake up every day and build better lives for themselves, their families, and their communities, regardless of the things they lack or the obstacles they encounter. As Jackley puts it, “The greatest entrepreneurs succeed not because of what they possess but because of what they are determined to do.” In Clay Water Brick, Jackley challenges readers to embrace entrepreneurship as a powerful force for change in the world. She shares her own story of founding Kiva with little more than a laptop and a dream, and the stories and the lessons she has learned from those across the globe who are doing the most with the least. Praise for Clay Water Brick “Jessica Jackley didn’t wait for permission to change the world—she just did it. It turns out that you can too.”—Seth Godin, author of What to Do When It’s Your Turn “Fascinating . . . gripping . . . bursting with lessons . . . Jessica Jackley has written a remarkable book . . . so thoroughly well meaning and engagingly put it is too magnetic to put down.”—Financial Times “Clay Water Brick is a tremendously inspiring read. Jessica Jackley, the virtuoso co-founder of the revolutionary microlending platform Kiva, shares uplifting stories and compelling lessons on entrepreneurship, resilience, and character.”—Adam Grant, author of Give and Take “A blueprint for anyone who wants to make the world a better place and find fulfillment in the process, no matter how scarce their resources or how steep the challenge.”—Arianna Huffington “This book is inspirational. And honest and practical. . . . Well written, thoughtful: a selfless account of how to succeed by doing right and following your heart.”—Booklist |
