Mulligan goes to work at an HMO and records what it’s really like to manage care. Set at a health insurance company dubbed Acme, this book chronicles how the privatization of the health care system in Puerto Rico transformed the experience of accessing and providing care on the island. Through interviews and participant observation, the book explores the everyday contexts in which market reforms were enacted. It follows privatization into the compliance department of a managed care organization, through the visits of federal auditors to a health plan, and into the homes of health plan members who recount their experiences navigating the new managed care system. In the 1990s and early 2000s, policymakers in Puerto Rico sold off most of the island’s public health facilities and enrolled the poor, elderly and disabled into for-profit managed care plans. These reforms were supposed to promote efficiency, cost-effectiveness, and high quality care. Despite the optimistic promises of market-based reforms, the system became more expensive, not more efficient; patients rarely behaved as the expected health-maximizing information processing consumers; and care became more chaotic and difficult to access. Citizens continued to look to the state to provide health services for the poor, disabled, and elderly. This book argues that pro-market reforms failed to deliver on many of their promises. The health care system in Puerto Rico was dramatically transformed, just not according to plan. |

However, we lack comparative studies of these participatory institutions in public health. This paper analyzes the extent to which workers in Bolivia face barriers to entry in the formal and informal sectors of the urban labor market. These barriers are most prevalent in the formal sector because of regulation. The higher wages found in the that sector are often regarded as evidence of labor market segmentation. However, wage differences between sectors may also result from compensating wage differentials, which follow from non-monetary returns to the job such as health insurance, utility associated with the workplace, and job security. The author proposes a model that allows testing for labor market segmentation between the two sectors on the basis of cross- sectional data. The methodology incorporates data on ways in which individuals search for new jobs and information about discouraged workers who have stopped searching for jobs. The proposed model accounts for all of the specific features of urban labor markets in developing countries, in particular the existence of a competitive informal sector. |

Grey, 2002-05-24 In New Deal Medicine, physician and historian Michael Grey brings to light the diversity, reach, and complexity of the medical care programs of the Farm Security Administration. Drawing on oral histories, archival records, and medical journals from the 1930s and 1940s, Grey finds the programs were both a rehearsal for more modern forms of medical organization and a lightning rod for critics of socialized medicine. He assesses the compromises made to try to preserve the programs' somewhat secret objective of providing the poor with health care while not running afoul of conservative politicians and their colleagues in the AMA. Acknowledging the effect of changing demographics (doctors, nurses, and farmers alike marched off to war) and economics, Grey contends that these factors do not fully explain the demise of the FSA experiment in health care. Rather, the political winds shifted at the same time that the medical profession acted to protect its authority over the practice of medicine. New Deal Medicine shows that, by the peculiarly American style of incrementalism, many of the FSA medical care structures and goals have been at least partially realized in the United States and in Canada. The lessons learned by the FSA personnel were transferred into health programs in Canada, in the labor unions, and finally in Lyndon Johnson's Great Society. |
