Bray, 1996-06-19 The fact that about 30-40% of the adults in the Western world are overweight or obese testifies to the frequency of the disturbances in body weight regulation. Scientists have established that caloric intake, macronutrient composition of the diet, basal and resting metabolic rate, thermic response to food, energy expenditure associated with movement and physical activity, and preferential storage of the surplus of calories as fat or lean tissues are critical determinants of energy balance and body weight. While much has been learned, the field is poised for major advances with the advent of a variety of imaging techniques, progress in quantitative and molecular genetics, use of transgenic rodent models and of breeding experiments with informative inbred strains, availability of stable isotopes for metabolic and behavioral studies, and a growing number of useful experimental animal and human models. This volume takes an integrative approach to obesity. It is structured around four major topics: (1) the animal and human models currently available for the study of body weight regulation with their strengths and limitations, (2) the molecular and genetic basis of the regulation of body weight, (3) the metabolic and physiological mechanisms involved, and (4) the behavioral and social determinants. The 13 background papers provide a critical overview of the present knowledge base while the group reports summarize the extensive deliberations of 38 international experts. The objectives of the program were to examine the role of the endocrine system in a wide spectrum of adjustments and adaptations to changes in environmental conditions by various spe cies of animals, including man, and to promote an ex change of ideas among investigators who have approached these functions from diverse aspects. The diversity of the information and ideas communicated is great. Of necessity, they represent only an extremely modest se lection of the many facets of endocrine function in the interaction of animals with their environments. For decades the agency has dealt with questions about what animal-testing data to use to make predictions about human health hazards, how to perform dose-response extrapolations, how to identify and protect susceptible subpopulations, and how to address uncertainties. As alternatives to traditional toxicity testing have emerged, the agency has been faced with additional questions about how to incorporate data from such tests into its chemical assessments and whether such tests can replace some traditional testing methods. Endocrine active chemicals (EACs) have raised concerns that traditional toxicity-testing protocols might be inadequate to identify all potential hazards to human health because they have the ability to modulate normal hormone function, and small alterations in hormone concentrations, particularly during sensitive life stages, can have lasting and significant effects. To address concerns about potential human health effects from EACs at low doses, this report develops a strategy to evaluate the evidence for such low-dose effects. |

In the United States, endocrinologists have been providing hormonal therapy for transgender individuals for decades; however, until recently, there has been only limited literature on this subject, and non-endocrine aspects of medical care for transgender individual have not been well addressed in the endocrine literature. While this type of information on high-resource countries is readily available, Cancer Survival in Africa, Asia, the Caribbean and Central America presents in-depth cancer survival data from 27 population-based cancer registries in 14 low- and middle-resource countries. The striking inequalities in cancer survival between countries and within countries described in this volume are largely related to the differences in general awareness, availability of early detection practices, trained human resources, diagnosis and treatment and the development and accessibility to cancer services, as well as, to a lesser extent, to issues of data quality and reliability. The differences in cancer survival reported in populations observed between and within countries studied in this volume provide valuable insights for future planning and investment by governments in primary prevention activities, early detection initiatives and tertiary care to achieve meaningful cancer control. The calendar period of registration of incident cases for the present study ranges between 1990 and 2001. Data on 564 606 cases of 1-56 cancer sites from different registries are reported. Data from eleven registries were utilized for eliciting survival trend and seventeen registries for reporting survival by clinical extent of disease. Besides chapters on every registry and general chapters on methodology, database and overview, the availability of online comparative statistics on cancer survival data by participating registries or cancer site in the form of tables or graphs is an added feature. |
