hospital discharge papers: Facilitating Patient Understanding of Discharge Instructions Institute of Medicine, Board on Population Health and Public Health Practice, Roundtable on Health Literacy, 2014-12-01 The Roundtable on Health Literacy brings together leaders from academia, industry, government, foundations, and associations and representatives of patient and consumer interests who work to improve health literacy. To achieve its mission, the roundtable discusses challenges facing health literacy practice and research and identifies approaches to promote health literacy through mechanisms and partnerships in both the public and private sectors. To explore the aspects of health literacy that impact the ability of patients to understand and follow discharge instructions and to learn from examples of how discharge instructions can be written to improve patient understanding of-and hence compliance with-discharge instructions, the Roundtable on Health Literacy held a public workshop. The workshop featured presentations and discussions that examined the implications of health literacy for discharge instructions for both ambulatory and inpatient facilities. Facilitating Patient Understanding of Discharge Instructions summarizes the presentations and discussions of the workshop. This report gives an overview of the impact of discharge instructions on outcomes, and discusses the specifics of inpatient discharge summaries and outpatient after-visit summaries. The report also contains case studies illustrating different approaches to improving discharge instructions. |

hospital discharge papers: Ask Me About My Uterus Abby Norman, 2018-03-06 For any woman who has experienced illness, chronic pain, or endometriosis comes an inspiring memoir advocating for recognition of women's health issues In the fall of 2010, Abby Norman's strong dancer's body dropped forty pounds and gray hairs began to sprout from her temples. She was repeatedly hospitalized in excruciating pain, but the doctors insisted it was a urinary tract infection and sent her home with antibiotics. Unable to get out of bed, much less attend class, Norman dropped out of college and embarked on what would become a years-long journey to discover what was wrong with her. It wasn't until she took matters into her own hands -- securing a job in a hospital and educating herself over lunchtime reading in the medical library -- that she found an accurate diagnosis of endometriosis. In Ask Me About My Uterus, Norman describes what it was like to have her pain dismissed, to be told it was all in her head, only to be taken seriously when she was accompanied by a boyfriend who confirmed that her sexual performance was, indeed, compromised. Putting her own trials into a broader historical, sociocultural, and political context, Norman shows that women's bodies have long been the battleground of a never-ending war for power, control, medical knowledge, and truth. It's time to refute the belief that being a woman is a preexisting condition. |

hospital discharge papers: The Computer-Based Patient Record Institute of Medicine, Committee on Improving the Patient Record, 1997-10-14 Most industries have plunged into data automation, but health care organizations have lagged in moving patients' medical records from paper to computers. In its first edition, this book presented a blueprint for introducing the computer-based patient record (CPR). The revised edition adds new information to the original book. One section describes recent developments, including the creation of a computer-based patient record institute. An international chapter highlights what is new in this still-emerging technology. An expert committee explores the potential of machine-readable CPRs to improve diagnostic and care decisions, provide a database for policymaking, and much more, addressing these key questions: Who uses patient records? What technology is available and what further research is necessary to meet users' needs? What should government, medical organizations, and others do to make the transition to CPRs? The volume also explores such issues as privacy and confidentiality, costs, the need for training, legal barriers to CPRs, and other key topics. |

hospital discharge papers: Better Faster Farther Maggie Mertens, 2024-06-18 “From foot-binding to corsets, patriarchal societies have found ways to immobilize women, but now, marathoners and Olympians are proving that women can run like the wind!” —GLORIA STEINEM A look behind the curtain that all women who love running and sport should read.” —KARA GOUCHER, Olympic runner and New York Times-bestselling author of The Longest Race More than a century ago, a woman ran in the very first modern Olympic marathon. She just did it without permission. Award-winning journalist Maggie Mertens uncovers the story of how women broke into competitive running and how they are getting faster and fiercer every day—and changing our understanding of what is possible as they go. Despite women proving their abilities on the track time and again, men in the medical establishment, media, and athletic associations have fought to keep women (or at least white women) fragile—and sometimes literally tried to push them out of the race (see Kathrine Switzer, Boston Marathon, 1967). Yet before there were running shoes for women, they ran barefoot or in nursing shoes. They ran without sports bras, which weren’t invented until 1977, or disguised as men. They faced down doctors who put them on bed rest and newspaper reports that said women collapsed if they ran a mere eight hundred meters, just two laps around the track. Still today, women face relentless attention to their bodies: Is she too strong, too masculine? Is she even really a woman? Mertens transports us from that first boundary-breaking marathon in Greece, 1896, to the earliest “official” women’s races of the twentieth century to today’s most intense ultramarathons, in which women are setting all-out records, even against men. For readers of Good and Mad, Born to Run, and Fly Girls, Better Faster Farther takes us inside the lives and the victories of the women who have redefined society’s image of strength and power. An essential read to normalize women's existence, excellence, and humanity within the sport of running.” —ALISON MARIELLA DÉSIR |
