Between these two pieces, the contributors, all highly acclaimed for their clinical, theoretical or research work, present original, cutting edge work on this complex subject. Fortunately, with the continued commitment to explore with others, some of these perspectives will remain in flux and new understanding will emerge. In the meantime, this text may serve to reinforce those willing to quietly integrate the panorama and acknowledge all who have been a part of their own journey.
